Please send this report duly completed and signed to your National Agency within 30 calendar days after the end of the activity. Once this report and the supporting documents (e.g. a certificate of attendance or, in the case of job shadowing, a signed letter from the host organisation) are submitted and approved, the National Agency will either pay the balance of the grant or recover any unspent funds.	Queira preencher na íntegra este relatório final de avaliação e devolva-o datado e assinado, no prazo de 30 dias após a conclusão da actividade de formação. Após a recepção e aprovação, por parte da Agência Nacional, deste relatório final e respectivos anexos (cópia do certificado de participação na actividade de formação ou, no caso de job-shadowing, uma carta assinada pela instituição de acolhimento; e, se aplicável, cópia do comprovativo de realização de preparação linguística), será dada a ordem de pagamento do saldo remanescente da subvenção ou a ordem de reembolso do montante devido pelo beneficiário relativo a despesas não efectuadas.
